Today I ran across a Health Question Form from Summit Health Products which is one of the best free online health questionnaires I have seen. It covers everything from your diet to your mental/spiritual state, sleep habits and overall lifestyle. The questionnaire also has you state five lifestyle changes/goals so that it can evaluate your health based upon what your lifestyle goals are.
